We use email to correspond with patients and prospective patients because it remains a record of the questions and concerns asked and the answers given. The information you provide gives us the information we need to determine how best we can serve you. If you are corresponding for the patient please give the best contact information, yours or theirs. All other answers should refer to the person you are assisting.
